Dr. Nadav Fields - Medical Director
Dr. Nadav Fields - Medical Director
Dr. Nadav Fields is the co-founder and medical director of CR8 Health. Dr. Fields received his bachelor’s degree from the University of Florida and completed his master’s degree in Biomedicine at Florida Atlantic University. He graduated from Touro College of Medicine New York and completed his residency training in Internal Medicine at the University of Arizona.
He is double board-certified by the American Board of Internal Medicine and the American Board of Obesity Medicine. Dr. Fields is trained in Bioidentical Hormone Replacement Therapy by the Preventative & Innovative Medicine Academy. Additionally, he is certified in Peptide Therapy and completed training with Dr. William Seeds, the world’s leader in Cellular and Peptide Therapy.
“Throughout his career, Dr. Nadav has worked in the emergency department, general medical ward, and in intensive care unit settings. It was here that he witnessed the sequelae of poor nutrition and lifestyle choices, which resulted in accelerated aging, obesity, heart disease, strokes, and premature death.
With growing frustrations in our healthcare system where we mostly focus on treating current disease states and alleviating symptoms, Dr. Fields became increasingly interested in Preventative Health with a focus on proactive not reactive medicine. This path eventually led him to opening CR8 Health where he can now engage, educate and empower his patient’s to reach their health destiny.”

Dr. Jonathan Fields was first drawn to studying Holistic and Eastern medicine over 20 years ago via his lifelong study of martial arts. After suffering for over a year with an undiagnosed and debilitating illness in his early thirties, and being completely let down by the conventional healthcare system, he was able to make a full recovery without any pharmaceuticals or surgeries. Being grateful for his second chance, and now having the ability to empathize with patients in dire need, he dedicated the rest of his life to empowering others with similar stories.
Dr. Jonathan is also a published author, public speaker, entrepreneur, martial arts, tai chi & meditation instructor, transformational retreat host, and plant medicine advocate. He owns multiple integrative practices in South Florida, lectures at Universities & Fortune 500 companies, works with elite athletes & celebrities, and has formulated for publicly traded companies.

Jena Brusilovsky APRN-C is a board-certified nurse practitioner specializing in Adult Primary Care and Aesthetic Medicine. With over 10 years of experience in aesthetic medicine, Jena’s passion lies in enhancing one’s natural beauty. With an extensive background in plastic surgery and specialized aesthetic training, Jena brings a unique blend of medical expertise and artistry to every patient. She is committed to staying at the forefront of aesthetic medicine to provide personalized outcomes and effective treatments to help clients look and feel their best.
Jena is a wife and mom of two kids. She has lived in New York City and Chicago and has recently relocated her family to Florida. Jena will be specializing in aesthetics as well as weight loss and hormones here at CR8 Health.

Isabella Fields is the General Manager of the CR8 Health office in Boca, bringing her expertise in Business Administration with a focus on Marketing to the team.
Originally from the Dominican Republic, she is passionate about creating a warm, family-like atmosphere where every patient feels genuinely cared for, heard, and supported. With a strong commitment to exceptional customer service, Isabella ensures that each patient receives personalized attention, making their experience as comfortable and seamless as possible.
